2015 Community Health Need Assessment
All four MHS hospitals, Memorial Medical Center, Abraham Lincoln Memorial Hospital, Taylorville Memorial Hospital and Passavant Area Hospital, are completing community health need assessments in 2015. Complete information on the assessments will be shared when they are completed.
As part of its assessment, Memorial Medical Center, St. John’s Hospital, Sangamon County Department of Public and SIU School of Medicine collaborated with the University of Illinois at Springfield to conduct a Sangamon County community health survey and five community forums in October 2014. Results from the survey and forums are posted below.

A multitude of issues combine to determine the health of a community. In addition to diseases, community health is affected by substance abuse, educational attainment, economic status, environmental issues and the personal choices of the people who make up that community. No one single hospital, agency or governmental body can be entirely responsible for the health of its community. No organization can address the multitude of issues alone.
